Long Term Care Virtual Care Program
In August 2022, THP partnered with Mississauga’s Long-Term Care (LTC) homes to launch the LTC Virtual Care program. This initiative aims to improve specialist care and navigation support for LTC homes.
Key Achievements
- Successfully implemented in 14 out of 15 LTC homes in Mississauga.
- Provides specialized care to residents in the comfort of their LTC homes.
- Prevents avoidable Emergency Department (ED) visits and eliminates prolonged ED wait times and avoids exposure to nosocomial infections
- Reduces workload for LTC staff by minimizing hospital transitions.
- Lowers overall care costs by reducing unnecessary hospital transfers and ED visits.
Virtual Care Pathways
- Virtual Phone Consultations: Direct communication between LTC clinicians and General Internal Medicine (GIM) specialists.
- Virtual Video Visits: Comprehensive care assessments through a virtual video consultation between LTC residents and physician specialists.
- Diagnostic Imaging Pathway: Timely access to diagnostic imaging services, such as X-Ray, Ultrasound, CT and MRI.
- Navigation Support: Helps LTC homes find and access relevant clinical resources within the Hospital or community.
This program highlights a commitment to innovation in healthcare, enhancing patient outcomes, improving access to specialized care, and optimizing resource use in the LTC setting.
When to Contact LTC Virtual Care:
LTC Staff can reach out to LTC Virtual Care in the following situations:
- When considering the transfer of a resident with urgent medical issues to the hospital.
- If a resident requires a visit with a General Internal Medicine (GIM) specialist (on-call).
- When uncertain about how to manage an LTC resident’s acute clinical change.
- If a resident’s family requests an Emergency Department (ED) visit for their loved one, but you believe the issue can be addressed virtually.
- When assistance is needed in locating a specialist or outpatient clinic.
- If guidance is required in navigating community and hospital resources.
